Preparing for your appointment

We respect your decision to take time for your healing and want to ensure your comfort. Please prepare to dress down for your treatment or wear loose clothing. Ensure you are hydrated before and after your treatment. Please notify us 24-hours in advance if you cannot keep your appointment to avoid a $60 fee.

Send us a picture of the front and back of your insurance card, include your address and date of birth to hello@vitalizeacupuncture.com.  We will do our best to confirm your benefits and email you before your first visit.

We require at least 48 hours in order to verify your insurance benefits. You may also call your insurance to inquire about your alternative care benefits in the event that you need to schedule a last minute appointment but have questions about your payment responsibilities.

Kat Starbird is currently out of network with some carriers listed above, call to inquire.

Chiropractor

In network with all major carriers. We are unable to except Kaiser/CHP or OHP at this time.

Massage Therapy

In network with all major carriers. We are unable to except Kaiser/CHP, ASH, CIGNA, AETNA and Health Net at this time.
